=== Intestinal colonization and sepsis === Enterohemorrhagic E. coli (EHEC) and enteropathogenic E. coli (EPEC) are pathogens that rely on OmpT to colonize in the intestine of their host. In response to the presence of E. coli in the gut, the host releases antimicrobial peptides as part of the innate immune response. Since OmpT can break down these antimicrobials and inactivate them, EHEC and EPEC can colonize within the colon or small intestine of the host and lead to serious diarrheal diseases. In the case of sepsis, the host activates the blood clotting system to deposit fibrin and limit the spread of bacteria throughout the blood. Additionally, it appears that benzodiazepines metabolized by nitro-reduction (clonazepam, nitrazepam) may also, in a somewhat similar vein, be unlikely to be affected by fluvoxamine. Using fluvoxamine and alprazolam together can increase alprazolam plasma concentrations. If alprazolam is coadministered with fluvoxamine, the initial alprazolam dose should be reduced to the lowest effective dose. As with all SSRI medications, using fluvoxamine with NSAIDs like ibuprofen may increase the risk of bleeding, particularly in the GI tract. Fluvoxamine is contraindicated with other medications that increase serotonin (dextromethorphan, ondansetron, amphetamine, sumatriptan, Tramadol, Hypericum perforatum, etc.). Combining these medications may rarely lead to a life-threatening complication known as serotonin syndrome. Fluvoxamine and ramelteon coadministration is not indicated. Fluvoxamine has been observed to increase serum concentrations of mirtazapine, which is mainly metabolized by CYP1A2, CYP2D6, and CYP3A4, by three- to four-fold in humans. Caution and adjustment of dosage as necessary are warranted when combining fluvoxamine and mirtazapine. Fluvoxamine seriously affects the pharmacokinetics of tizanidine and increases the intensity and duration of its effects.
HIV infection In October 2016, scientists from Emory University and National Institute of Allergy and Infectious Diseases (NIAID) published a paper which claimed that they applied daily ART (antiretroviral therapy) of 90 days followed by simianized (rhesus macaques) anti α4β7 antibody on SIV+ rhesus macaques for 23 weeks. Twenty three months after stopping both ART and anti-α4β7 antibody treatment, the in vivo SIV level still remained undetectable. Therefore, treating HIV+ people with ART and anti-α4β7 simultaneously may be a new therapy that could potentially lead to an HIV infection cure. In mice, vedolizumab was not able to prevent or control HIV-infections. Phase 1 clinical trial of that therapy has been initialized by NIAID since May 2016. For each of the participants, they will get vedolizumab infusions every four weeks for 30 weeks. Before the 23rd week of vedolizumab infusions, cART (combination ART) is kept. During the 30 weeks, blood draws are repeated for baseline tests. After the 22-week-cART is stopped, both viral load and CD4 count will be monitored biweekly. If HIV viral load goes high or their CD4 cell counts decrease by too much when vedolizumab is used alone, cART will be brought back on the participants. The published results from this clinical trial suggest "that blockade of α4β7 may not be an effective strategy for inducing virological remission in HIV-infected individuals after ART interruption" because only one patient showed prolonged virus suppression.
